Machine maintenance turned deadly when a worker’s neck was sliced by a machine he was fixing, Texas police said.

David Mata, 46, was conducting maintenance on a machine at the Cuisine Solutions plant on June 7 when he was pinned inside it, San Antonio police and KSAT reported.

The machine reached his neck, causing a deadly laceration, police said.

Individuals attempted to save him, according to police, but he eventually died of his injuries. The Bexar County Medical Examiner’s Office determined he died of head and neck trauma, KENS reported.

“We are working alongside local authorities and following all OSHA US Department of Labor’s protocols as we assess this incident,” Cuisine Solutions said in a statement to KSAT.

Police are continuing to investigate the incident. – The Charlotte Observer/Tribune News Service
